Some common mistakes in HAZOPs

by Dr. Arturo Trujillo
Hazard and Operability Study (HAZOP) is one of the most powerful tools for the identification, semiquantification, and mitigation of risks in process plants. It has been increasingly used both as a tool for risk assessment and for prioritizing actions for risk mitigation. Unfortunately, due to its time-consuming nature, HAZOP requires that organizations invest a great deal of resources to its execution. This article addresses solutions and safeguards to the most common HAZOP mistakes, including:
• Improper planning for HAZOPs
• Disorganized HAZOsP
• Poorly prepared HAZOPs
• Limited or minimalist HAZOsP
